If you are currently receiving accommodations at St. Vincent High School, you may be eligible for accommodations on the PSAT, SAT, AP and/or ACT tests. These accommodations requests must be made early as it can take up to 7 weeks for accommodations to be approved.
For information on requesting accommodations, please contact: Mrs.Gina Jacques SVHS Resource Specialist [email protected]
The following documentation needs to be completed and returned to your counselor before any request can be submitted. Submission of this form officially starts the request process.
You are requesting accommodations before the written deadline:
The signed consent form must be submitted to your counselor at least ten business days before the official Collegeboard request deadline in order to guarantee its timely completion. The most up to date information regarding submission deadlines can be found here: Testing Accommodation Calendar
NEXT STEPS: Once your consent form is received, your resource counselor will use the information in your IEP/504 to complete the request. This means that all accommodations will match the test accommodations portion of the official plan. If you feel that an accommodation is missing or incorrect, please connect with your resource counselor prior to submitting the consent form.
In the event that Collegeboard asks for additional information (historically this has been to substantiate rare conditions and/or newly implemented plans), the most up to date testing completed but SVHS will be submitted. Families are responsible for supplying any additional information, including outside testing and recommendations, in the case that more documentation is requested.
PLEASE NOTE: In most situations, when school accommodations have been effectively used for lengthy amounts of time, Collegeboard will approve accommodation requests received from counselors without the need of additional information. However, for new accommodations or less commonly requested accommodations, they may require that we submit documentation. The approval process can take up to seven weeks. You will receive communication directly from Collegeboard with their response as well as information from us regarding next steps.
REQUIREMENTS: 1. The following documentation needs to be completed and returned to your counselor before any request can be submitted. Submission of this form officially starts the request process.
2. You are requesting accommodations before the written deadline:
The signed consent form must be submitted to your counselor at least ten business days before the official ACT request deadline in order to guarantee its timely completion. The most up to date information regarding submission deadlines can be found here (the accommodations deadline is currently the same as the national registration deadline): ACT Registration
NEXT STEPS: The accommodations request process begins when you are ready to register for your ACT test date. When registering, ACT will ask you if you would like to request accommodations you use in school for the ACT exam. After you select YES, and submit your registration and payment, the Counseling Department will receive an email directly from ACT, which will initiate the request process. You are also welcome to email your resource counselor to let them know you have registered for the ACT and would like to begin the accommodation request process.
Once we have the consent form on file, and the official ACT email is received, your resource counselor will use the information in your IEP/504 to complete the request. This means that all accommodations will match the test accommodations portion of the official plan. If you feel that an accommodation is missing or incorrect, please connect with your counselor prior to submitting the consent form.
ACT always asks for documentation to substantiate the accommodation request, regardless of the disabilities or the length of time the plan has been in place. We will automatically submit the most up to date testing completed by SVHS. Families are responsible for supplying any additional information, including outside testing and recommendations. PLEASE NOTE: In most situations, when school accommodations have been effectively used for lengthy amounts of time, ACT will approve accommodation requests. However, for newer accommodations or less commonly requested accommodations, they may require additional documentation. The approval process usually takes under a month. You will receive communication from us with the official ACT response as well as information regarding next steps.
